How to Apply: 5 Registration Steps
Complete these five steps, in order, to join our team of wound care dietitians.
Step 1 – Hold an active state dietitian license. If you are not already licensed, register with your state as a licensed dietitian before moving on.
Step 2 – Obtain a National Provider Identifier (NPI). Apply through the National Plan and Provider Enumeration System (NPPES): nppes.cms.hhs.gov
Step 3 – Complete the Credentialing Form (Group Practice). Complete the Berry Street credentialing form here. Email woundcaredietitians@gmail.com to request a copy of our group insurance policy — every dietitian on our team is covered under the practice’s policy, so you do not need to purchase your own.
Step 4 – Provide your bio and final details (Berry Street). Submit your provider bio and final details here.
Step 5 – Confirm your application with Wellness & Wound Care Dietitians. Use the confirmation form below to send us your first name, last name, email, phone, LinkedIn profile, why you want to work with Wellness & Wound Care Dietitians, a link to your profile picture, and a short bio.
What happens next: once all five steps are complete, we will reach out by email to finalize the details, including payroll activation and your contract. The full process typically takes 30–60 days — thank you for your patience and persistence!
Confirm Registration and Request our Insurance Certificate for Credentialing
Good news: you will not need to purchase or carry separate insurance to work with us. As a group practice, every dietitian on our team is covered under the Wellness and Wound Care Dietitians, LLC umbrella policy.
Confirm below that you have completed Steps 1 through 4, and we will send our Certificate of Insurance to Berry Street on your behalf so your credentialing can move forward.
